Healthwatch Northumberland has launched its annual survey to gauge public feedback on health and social care experiences.
The organisation wants to know how people would rate their experiences over the last 12 months, and whether they think services have improved or got worse.
People's experiences will be fed back to those paying for and providing services.
The findings will help set priorities for the forthcoming year.
Healthwatch Northumberland project coordinator, Derry Nugent, stated: "The strength of Healthwatch Northumberland is that we focus on the issues that are most important to the people of Northumberland and where we can help make a difference.
"We are encouraging everyone to have their say on what this should be this year.
"We are eager to see what you choose."
The survey, available on its official website and via post, welcomes responses from both patients and carers.
The deadline for responses is May 6.
