No 22, a restaurant, cafe and canteen at 22 Front Street, Prudhoe, Northumberland, was given the score after assessment on September 29, the Food Standards Agency's website shows.
It means that of Northumberland's 634 restaurants, cafes and canteens with ratings, 556 (88 per cent) have ratings of five and just one has a zero rating.
The four-out-of-five rating means the restaurant's food hygiene levels are 'good'.
The rating includes:
- Handling of food

- How food is prepared
- Cleanliness of facilities
- How food safety is managed.
