The 22-year-old professional, from Wark, scored in the third minute of the League Two play-off final against Exeter, on May 28.
And he provided the cross for Mark Cullen to score the winner in a dramatic 2-1 victory at Wembley.
It was a memorable day for the midfielder, who was watched by friends and family members from the stands.
Granddad Les Telfer said: “For a young lad from Wark to go to Wembley, score the first goal and then set up the winner, is absolutely fantastic, and we are all very proud.
“Brad has already played at League One level, and we just hope he stays injury free and can have another good go at it next season.”
Potts, who formerly attended Haydon Bridge High School, has been a revelation since joining Blackpool from Carlisle United in 2015.
In March, he was named in the Football Manager League Two Team of the Season after weighing in with 12 goals and nine assists to his name at the time.
